The survey configuration attempts (weakly) to defeat spamming by allowing only one response from a single computer. If a user has completed a survey (clicked "Done"), any subsequent attempt to re-enter the survey form will be met with an HTTP redirect action which takes the user to the OASIS Home Page. If you think something has gone wrong so that you cannot complete the survey, please send email to Robin Cover (OASIS Director of Information Services) with a description of the unexpected SurveyMonkey behavior.
